  • If you are applying for non urgent care assistance you must print out and complete the application, with
    proof of income and a witness’s signature. Mail the completed application to  ~ GPAC at PO Box 732,
    Wooster, Ohio, 44691.

  • Please be sure to PRINT. If we can't read your application we can't help you.

  • GPAC wants to help as many animals as we can. Please help us to help them by taking your time and
    following the process as the directions indicate. If you have a question about something on the GPAC
    financial application please send GPAC an email and we will do what we can to answer your question(s).
